Introduction/Rationale: Persons with chronic musculoskeletal pain (CMP) are restricted in their participation in the life area of work and employment. Therefore, we need a deeper understanding of context factors that support or hinder their participation, as a basis for designing, implementing, and evaluating occupational therapy and interprofessional interventions. Objective: To investigate which context factors influence the ability of persons with CMP to participate in the life area of work and employment. Methods/Approach: Nested case study using a realist evaluation framework. Qualitative content analysis of problem-centred interviews. Results and Practice Implication: We identified several context factors that influenced participation in the life area of work and employment of persons with CMP: In the pre-interventional phase, socioeconomic and environmental factors affected experiences of “exhaustion” and “discrimination”. In the intra-interventional phase, the social skills of occupational therapist and other health professionals and opportunities for discussion with peers affected the ability to engage in the interprofessional intervention programme. In the post-intervention phase, volitional competences of the social system (e.g., family and close friends) affected the sustainable application of the interprofessional intervention programme in different life areas, including work and employment. This knowledge has important implication for designing and implementing occupational therapy and interprofessional interventions to support participation of persons with CMP. Conclusion: The ability of persons with CMP to participate in the life area of work and employment is influenced by context factors and in interaction with the ability to participate in other areas of life.de_CH
