Abstract: | Background and aim: Pediatric prosthetic fitting practice for congenital forearm reduction deficiency is a controversial topic. Evidence-based occupational therapy decision-making is essential for optimal participation and occupational support. Therefore the goal of this review was to provide an overview of evidence-based criteria for occupational therapists regarding prosthetic fitting and alternative adaptive devices (AAD) and/or compensatory strategies for children with unilateral congenital transversal reduction deficiency below elbow (UCTRDBE). The focus was on children aged zero to 14 years. Methods: We conducted a systematized review. We critically appraised the seven included studies and summarized the results in the form of a decision-support flow chart. Results: Multiple facts influence clinical decision-making regards to prosthesis prescription. Prosthesis are especially accepted if they provide the user with clear functional or cosmetic benefits. In all other cases, AAD and/or teaching compensatory strategies have greater benefits. The social environment, especially parents, have a crucial influence on the attitude towards coping with disability and participation in activities of daily living (ADL) of the child. Conclusion: Prosthetics are particularly useful for specific activities during which they provide a clear functional benefit (e. g. cycling or eating). The potential of AAD and/or compensatory strategies should not be neglected during a comprehensive rehabilitation process. |
