Abstract:	Background: The effectiveness of physiotherapy to reduce low back pain depends on patients’ adherence to physiotherapy. Facilitators and barriers to patient adherence are multifactorial and include personal factors such as previous experiences, and therapist-related factors. No recommendations are currently available to guide physiotherapists how to best facilitate adherence in patients attending physiotherapy for low back pain. Method: International experts on adherence of patients with low back pain were invited to participate in a three-round standard Delphi survey. The survey contained 49 items (round 1: 32, round 2: 17) which were rated on 5-point Likert scales. The items were assigned to six dimensions. The consensus level was defined as 70 %. The consensus on each item was calculated using the mean values of ratings per item. In the third round, experts were asked about their agreement regarding the results from the first two rounds. Results: Of 38 invited experts, 15 followed the invitation and completed all three rounds. The consensus was reached on 89.81 % of the 49 proposed items to facilitate adherence. The highest consensus was achieved in the domains “Influence of cooperation between physiotherapists and patients” (99.19 %), and “Influence of biopsychosocial factors” (98.6 %). The lowest consensus was reached in the domains “The influence of administrative burdens” (75 %), and “Influence of digital-based therapy tools” (88.67 %). Conclusion: Results from this Delphi study show, that experts achieved a consensus on how to facilitate adherence of patients with low back pain. The main agreement was reached for biopsychosocial factors, therapeutic competencies, and the cooperation between patients and physiotherapists. The influence of administrative burdens and the potential influence of digital tools as add-on interventions is less clear. Future studies should prospectively evaluate the application of identified factors and evaluate in a longitudinal design their influence on patient adherence.
