dc.description.abstractBackground: In the day to day, nursing home residents with dementia do not always receive optimal care. The quality of care for these people may be especially inadequate at the end of life. To improve the level of care, there is a need to understand the process of nurses’ clinical decision-making in symptom management in the terminal phase of people with dementia.Aim: The aim of the study was to examine the decision-making process around symptom management for people with dementia in nursing homes in the terminal phase of life. Research Method: A qualitative design was chosen with a constructivist approach according to Charmaz. Individual interviews were conducted with nurses (n=32) in four Swiss nursing homes. Research Results: “Making sense“ of “changes within change“ plays a vital role in the care of people with dementia in the terminal phase, enabling symptom recognition and facilitating symptom management. Through a complicated process of interpretation or “finding out“, an understanding is reached of what, when and how changes have taken place. This process requires a continuous recounting and reflection of perceptions and observations. Highly-trained nurses are needed to help reduce the symptom burden experienced by people with dementia by supporting nursing staff through counseling, coaching and leadership. To optimize symptom management, nursing staff need opportunities for learning and discussion in order to explain changes, to develop courses of action and to allow for collective use of knowledge.de_CH
